Here is How It Works


1. Print, complete, and fax the "ONE TIME"
Contract / Payment Authorization Form to
1-(940) 766-2265 
OR 
print, complete, and email
 the Form to  txhomebase@yahoo.com 

 *This form must be received before a temporary
address can be assigned.*

2. Texas Home Base will give you a temporary address.

3. Have your product sent to the temporary address
assigned to you by Texas Home Base.

4. Texas Home Base receives your product and contacts
you with shipping charges.

5. Texas Home Base forwards product to the address
you have supplied.

Cost is $10.00* handling fee plus shipping charges to
your forwarding address.

* Per shipment received at Texas Home Base
if you receive 2 boxes, the charge is $20.00,
3 boxes charge is $30.00 etc. etc.
