1. Print, complete, and fax the "ONE TIME" Contract / Payment Authorization Form to 1-(940) 766-2265 OR print, complete, and email the Form to txhomebase@yahoo.com
*This form must be received before a temporary address can be assigned.*
2. Texas Home Base will give you a temporary address.
3. Have your product sent to the temporary address assigned to you by Texas Home Base.
4. Texas Home Base receives your product and contacts you with shipping charges.
5. Texas Home Base forwards product to the address you have supplied.
Cost is $10.00* handling fee plus shipping charges to your forwarding address.
* Per shipment received at Texas Home Base if you receive 2 boxes, the charge is $20.00, 3 boxes charge is $30.00 etc. etc.
What Items Can Not Be Sent
Items that are prohibited from shipping are as follows:
Arms and Weapons
Firearms and parts thereof
Human Remains
Live Animals and Plants
Tobacco
Meat or Meat Products
Radioactive Materials
Lottery Tickets
Please do not send any of these items to Texas Home Base to be forwarded.
When they are identified, Texas Home Base will return them to the original shipper.
The cost to return the package will be charged to your credit card or Paypal account.
